For athlete certification, a completed player registration includes: a current small photo of the child, the completed player registration form, a copy of the child's birth certificate,  a current physical and proof of grade*.  The child's name, squad, date of birth and date of exam should be highlighted on all forms. Teams should be organized in alphabetical order on the roster and in the binder/folder.  All documents must be out of page protectors for certification. 

Physicals must be dated 8/2/2023 or later to be used for the 2024 season. Physicals are required to state the athlete can participate without restrictions, is free from communicable disease and signed by a provider (FNP, PA, MD).

* Proof of grade (new 2024) - clubs will be required to submit proof of grade for each participate.  Acceptable documents include but not limited to: report card, class placement announcement, grade placement of school letterhead, print out from the school electronic system (must include date and assigned grade level) or other documents deemed approperiate by the FLYFCL Executive Board. 


For board/coach certification, a completed alphabetical roster is needed for the board and each individual team's coaching staff, a completed and notarized Affidavit and copies of all certifications (1st Aid/CPR/AED, USA Football, NAYS). All documents must be out of page protectors for certification.

INSURANCE DOCUMENTS 

Off-Season Insurance Requests: 
1) Complete the camp/clinic form and submit it to [email protected]
2) The Executive Board will review the form, collect any required additional information, then submit it for approval from the insurance company
3) 
Upon approval from the insurance company,  a COI will be issued and an invoice created for the additional coverage
4) The submitting club
 will pay the  invoice to FLYFCL  (due within 45 days from the date of the invoice)


In-Season Insurance Claim Submission: 
 
  1) Complete the FLYFCL accident report and the insurance claim form; email both documents to [email protected] 
2) Provide any additional information requested by FLYFCL or the insurance company
3) Collect and submit and follow up paperwork from the family of the injured athlete
*Ensure the family of the injured athlete is aware there is a $250 deductible for the FLYFCL insurance in addition to their own insurance policy deductible.
